Assessing Utilization for Detecting ICD Safety Differences

This study set out to evaluate whether the real-world usage of implantable cardioverter defibrillators (ICDs) is adequate to detect meaningful safety differences among various generator models. Using adverse event data from the American College of Cardiology’s ICD Registry (2006–2010), researchers calculated the device utilization required to observe statistically significant differences at 80% power. Three average adverse event rates, 3.9, 6.1, and 12.6 per 100 person-years, were tested against varying safety event rate ratios.

Findings indicated that small safety differences (rate ratio <1.15) are rarely detectable under current ICD utilization patterns, with only 34% or fewer models reaching the necessary threshold. For larger safety concerns (rate ratio ≥1.25), sufficient utilization was observed in up to 84% of models at higher event rates. However, uneven distribution of ICD usage across models limited the potential for comprehensive safety surveillance. The study highlights a critical need for enhanced post-market monitoring systems to ensure device safety.
